Legal Name Change

A Legal Name is the name that appears on your passport, driver’s license, birth certificate, or U.S. Social Security Card. To change the legal name, complete the Change of Biodemographic Info Form listing the current name on record, the updated legal name, and the reason for the change. If in changing the legal name, the person has changed gender they can update that on the form as well.

In addition to the form, please provide any supporting documentation such as a copy of court order, government issued identification, marriage certificate, etc. Make sure to sign and date it.

Your Legal Name will be used in business and other processes that require use of a Legal Name. These include:

  • Federal Requests for Information
  • National Student Clearinghouse
  • Transcripts
  • Academic Certifications
  • Diplomas
  • Athletics Rosters
  • Department of Public Safety

To change a legal name, complete the Change of Biodemographic Info Form.
